    We are seeking a detail orientated and dependable Registered Nurse to join our team!

    The Registered Nurse - Casual works one on one with clients through Clarity Care’s Home Care, Residential or Occupational Health Departments; focusing on our internal and external customer’s needs to help them achieve improved quality of life, employment and a sense of independence.

    Ideal Candidate Profile:
    • Licensed Registered Nurse (RN).
    • Experience with disability services preferred.
    • Strong clinical assessment skills.
    • Knowledge of state healthcare regulations.
    • Compassionate, person-centered approach.
    • Effective verbal and written communication.
    • Skilled in care planning.

    Key Responsibilities:
    • Plan, coordinate, and evaluate Member’s care.
    • Ensure regulatory and nursing compliance.
    • Conduct client assessments and transitions.
    • Obtain and renew physician orders.
    • Develop and update care plans.
    • Report significant condition changes.
    • Develop service reporting systems.
    • Train and instruct care staff.
    • Supervise, delegate, and evaluate staff.
    • Document consultations and nursing care.
    • Promote client independence and education.
    • Participate in employee health screenings and administering TBs.
    • Complete required PC documentation.

    Education and Qualifications:
    • Current licensure as a registered nurse under s.441.06, Wis. Stats.
    • Training and experience in the provision of personal care services or in a related program.
    • Undergo successful orientation and participate in competency-based assessments of skills on a regular basis as required by state law.
    • Must possess a valid WI driver’s license (where required), and ability to meet agency driving standards.  Must provide proof of auto insurance (where required).
    • Must pass a periodic WI Caregiver Background Checks including Department of Justice Criminal Record Checks, and/or that of previous states of residence.
