    Description

    The LGAR Health and Rehabilitation Center is a 4-star, 60 bed Skilled Nursing Facility nested in the hills of Turtle Creek in the eastern suburbs of Allegheny County. We are currently seeking a Full Time Registered Nurse, 1 st or 2nd shift with flexible shifts, to provide nursing care and nursing department supervision within the scope of the license, standards of care, and facility protocols.

    :Responsibilities:

    • Receive and give intershift report to RN Supervisor; provide necessary information to licensed and nurse aide staff; Assures the care plan is adhered to or revised when needed.
    • Complete assessments: admission and re-admission clinical assessments, change of condition, LOA return, incidents or accidents, post MD appointment or emergency deparment visit.
    • Communicate with physicians as needed about resident care needs, change of conditions; Inform on-call physicians of what care we can provide in facility; transcribe physician orders and execute them in aq timely manner.
    • Assure discharge preparations, discharge documentation, and discharge teaching is completed.
    • Documents in nurse's notes or other locations in accordance with standards of care and facility policies; Maintains clinical record in order.
    • Provide, superviser and assist with direct resident care; including but not liniti9ed to : call bell response, toileting, feeding, grooming, transfer, ambulation, restorative services;Implement evaluate and document behavior management; Direct LPN to perform their supervisor functions.
    • Complete physical, emotional and psychosocial assessments; Respond to changes in conditon; conduct regular rounds.
    • Perform resident admission, tranfer and discharge clinical responsibilies.
    • Monitor and assures clinical tracking documents are complete, including, but not limited to, I & O, food condumption, blood glucose, weiights, neuro checks, skilled and rehab needs, incident and qaccidents, pain management, Braden and other skin assessments, fall risk assessments.
    • Assure speciments are obtained, using correct technique, as prescribed.
    • Assure regular assessments are completed as needed: Complete weekly skin measurements and document according to facility protocol. Review weekly wound sheets Braden Scale; Complete weekly Falls and Pain assessments and document trends and follow up action, As assigned, provide information to the Director of Nursing who will presnt it to QA Committee.
    • Assure proper count and documentation of the controlled drugs.
