Job Requirements
  • Education Requirements: Nursing degree from an accredited program
  • Certifications Requirements: BLS certification, Valid state nursing license, EPIC proficiency
  • Experience Requirements: Minimum of 2 years of nursing experience, Hospice liaison experience preferred, Previous travel experience preferred
  • Other Requirements: Valid driver’s license with auto insurance, Comfortable with end-of-life symptom management, Ability to travel between hospitals and facilities, Strong communication and collaboration skills, Proficiency in electronic medical records (EPIC)

Job Responsibilities
  • Provide comprehensive end-of-life symptom management and patient education for hospice patients across multiple hospital settings and facilities.
  • Collaborate with physicians, facility staff, and family members to develop and implement appropriate hospice care plans tailored to individual patient needs.
  • Assess patients’ condition, deliver recommendations for hospice placement, and facilitate admissions both in hospital and home settings.
  • Travel between designated hospitals and facilities within the northern Green Bay/Oshkosh/Manitowoc/Sheboygan/Grafton area, providing timely and compassionate hospice services.
  • Maintain accurate documentation of patient interactions, assessments, and care recommendations using EPIC EMR system.
