    Registered Nurse - Home Health
    Location: Sterling Heights, MI
    Pay Rate: $62,000 to $80,000 per year

    Position Overview
    A compassionate and skilled Registered Nurse is being sought to join a growing home health team based in Sterling Heights, Michigan.     Key Responsibilities
    • Conduct comprehensive initial assessments and regular reassessments of patients in their home environments
    • Develop, implement, and continuously update individualized care plans in collaboration with the multidisciplinary care team
    • Administer medications, wound care, infusion therapy, and other skilled nursing interventions as ordered
    • Monitor and evaluate patient response to treatment plans and communicate changes to the supervising physician promptly
    • Educate patients and family members on disease management, medication adherence, safety practices, and self-care techniques
    • Accurately document all clinical activities using an electronic health record system in compliance with regulatory requirements
    • Coordinate with discharge planners and case managers to ensure smooth transitions of care
    • Participate in on-call rotations as required to support continuous patient care coverage
    • Maintain compliance with all applicable state, federal, and accreditation standards governing home health practice

    Requirements
    • Active and unrestricted Registered Nurse license in the state of Michigan
    • Minimum of one year of clinical nursing experience, with home health or medical-surgical experience strongly preferred
    • Current CPR and BLS certification
    • Valid Michigan driver's license and reliable personal transportation
    • Willingness and ability to travel to patient homes throughout the Sterling Heights and greater metro Detroit area
    • Strong clinical assessment and critical thinking skills
    • Proficiency with electronic health record documentation systems
    • Excellent communication, time management, and organizational abilities
    • Ability to work independently while maintaining strong collaboration with a care team

    Preferred Qualifications
    • Experience in a related field is preferred
