Position Overview:
We are seeking a
Registered Nurse (RN) to conduct
Start of Care (SOC) assessments and
60/90-day reassessments for our clients. This is an independent contractor (W9) position, offering flexible scheduling and rewarding work that directly impacts the well-being of those in need.
Responsibilities:
- Conduct in-home Start of Care (SOC) assessments for new clients.
- Complete 60-day and 90-day reassessments as required.
- Develop and review care plans to ensure appropriate non-medical support services.
- Communicate with caregivers and office staff regarding client needs.
- Maintain accurate and timely documentation.
Requirements:
- Active Registered Nurse (RN) license in the state of Ohio.
- Experience in home care, geriatrics, or community health is a plus.
- Ability to work independently and travel within the Cleveland area.
- Strong assessment, documentation, and communication skills.
- Reliable transportation and a valid driver’s license.
- Must be comfortable working as a W9 independent contractor.
Compensation & Benefits:
$40 - $75 per assessment based on assessment type.
$0.75 per mile reimbursement for travel.
