 Caregivers Home Health Services is looking to addmultiple RNs to their growing team of clinicians.

 Home Health Care Experience Required (Min. 2years)

COVID Vaccination, updated BLS, CPR, Neg. PPD REQUIRED

Skills Sets Preferred: Wound Care, Wound Vacs,IV's, PICC Lines, Tube Feeding, Trach Care, Foley Catheters, Ostomy Care, LabDraws, Case Management, CPR Certified

Major Areas of Responsibilities:

Patient and Family Educator: Educates thepatient and family on the disease process. Promotes and maintains patienthealth and independence through teaching and appropriate rehabilitativemeasures, while assisting patients in learning appropriate self-caretechniques. Advocates for patients and ensures participation in care planningand goals of care.

 Plan of Care: Assists patients through managingtheir Plan of Care and observes and monitors their conditions and performnecessary assessments. Performs patient comprehensive assessments andcollaborates with patient's multi-disciplinary care team (PT, OT, ST, MSW, andparaprofessionals) to develop, modify, and implement an individualized patientplan of care, per physician orders that ensure quality treatment, properdischarge planning, and achieves desired outcomes and goals.

 Documentation: Completes documentationtimely and accurately, including OASIS assessments, admissions,re-certifications, subsequent visits, physician orders, and care coordination.

Collaboration: Helps to achieve and maintaincontinuity of patient care by communicating information, including changes inthe plan of care, new orders, lab results, and new referrals promptly. Engagesother care team members when necessary to achieve desired patient outcomes.Facilitates interdisciplinary communication and follow-up through carecoordination. Participates in team meetings and case conferences and leadsdiscussions on multidisciplinary cases as appropriate. Maintains ongoingcommunication with a clinical manager regarding the progress of patients forassigned caseload.

Requirements

​​ Registered Nurse License in the State of Virginia

Home Health Care experience REQUIRED.

Prior EMR experience (Kinsser preferred)

MUST be efficient with documentation.
