Job Summary

Job Responsibilities
Interview patients to obtain past and present medical information and record data according to the format and procedures of Falls Community Health, recognize nonmedical needs of patients and their families, and make appropriate referrals to other agencies as necessary.
Assist attending physicians with medical services. Exhibit competency and perform clinic procedures according to policy, including, but not limited to, IV administration, EKGs, ear irrigations, spirometry, peak flows, and nebulizer treatments. Draw up and administer immunizations. Provide care under the direction of physician staff. Work as a member of a patient care team and provide patient-centered care. Conduct pre-visit and chronic disease care planning. Perform standing orders as approved by the Chief Medical Officer, including medication refills. Coordinate care with internal and external referrals. Initiate discussions on self-management goal setting. Participate with quality management work groups and teams to improve patient population health outcomes. Effectively communicate with representatives of diverse and vulnerable populations.
Perform physical assessment measurements and collect laboratory specimens.
Provide patients with information, education, and explanation regarding illness, treatment, medications, disease prevention, and follow-up care.
Assist with clinic supply inventory.
Attend in-service training, staff meetings, and workshops; utilize outside resources available to maintain professional nursing skills.
Perform other such duties and functions as are necessary and incidental to the proper performance of this position.
Minimum Qualifications
Graduation from an accredited registered nursing program with a current and valid licensure as an RN in the state of South Dakota. Minimum of one (1) year clinic or hospital nursing care experience required; or any such combination of education, experience, and training as may be acceptable to the hiring authority. Prior experience with electronic medical record systems is desirable.
Must possess or be able to obtain prior to hire a valid driver's license.

Ability to provide complete nursing services, both independently and as a member of the medical care team within the RN scope of practice.
Ability to serve and communicate with a wide variety of clients, including infants, youth, adults, low-income, and culturally diverse populations.
Ability to determine and accurately record medical data and service charges electronically while maintaining confidentially.
Ability and willingness to participate in training and informational meetings and functions.
Ability to think critically and appropriately analyze patient information and communicate efficiently with appropriate clinical staff.
Ability to communicate, both orally and in writing.
Ability to maintain confidentiality.
Knowledge of community agencies available for patient referrals.
