Responsibilities:- Conduct initial and ongoing patient assessments in clients’ homes
- Develop and review care plans based on patient needs
- Coordinate with caregivers and healthcare professionals
- Ensure accurate and timely documentationRequirements:
- Active RN license
- Experience in home care or clinical assessment preferred
- Strong communication and organizational skills
