InGenesis is currently seeking a Registered Nurse (RQR) to work with our client located in Columbus, Georgia. In this role, you will perform comprehensive assessments of patient healthcare needs, conduct physical and psychosocial evaluations, and provide essential medical care and support to patients transitioning from state hospital facilities to the community.

Key Responsibilities:

  • Assess patient healthcare needs through observations and diagnostic data.
  • Conduct physical and psychosocial evaluations, both virtually and in-person.
  • Perform routine tests and take vital signs.
  • Identify psychosocial stressors and document findings clearly.
  • Assist patients transitioning from state hospitals to the community within 48 hours.
  • Complete healthcare assessments based on various data sources.
  • Advise patients and families on health maintenance and disease prevention.
  • Work with an interdisciplinary team for developmental disability services.
  • Interview family members, caregivers, and primary care physicians.
  • Use health risk and functional assessment tools.
  • Provide clinical recommendations to support patient goals and needs.
  • Perform other duties as assigned.

Qualifications:

  • Registered Nurse (RN) license.
  • Experience in patient assessments.
  • Strong documentation and communication skills.
  • Ability to work well in a team.
  • Proficient with health assessment tools.
  • Valid driver's license with a clean record.
