    Registered Nurse (RN) – Transitional Care Management Clinic
    Location: Boardman, OH (Main Campus)
    Schedule: Full-time. Monday – Friday. Day shift.

    The Role
    You will assist patients recently discharged from acute care. You will gather clinical data and help prevent hospital readmissions.
     
    Key Responsibilities:
    • Patient Assessments: Conduct post-acute phone assessments. Identify barriers to patient recovery.
    • Care Coordination: Review discharge instructions, medications, and referrals.
    • Scheduling: Facilitate and schedule face-to-face follow-up visits.
    • Documentation: Log all patient calls accurately in the EMR system.
    • Compliance: Maintain patient privacy and strictly follow regulatory standards (OSHA, ODH).
     
    Qualifications
    • License: Active Ohio RN license.
    • Education: BSN degree is preferred.
    • Skills: Strong critical thinking and time management.
    • Communication: Clear, professional, and empathetic phone skills.
