    The Springs at Sherwood is seeking a compassionate, organized Registered Nurse to support our Health Services team. In this role, you will coordinate resident care, supervise and support caregivers and medication technicians, and serve as a key point of contact for residents, families, and staff.

    Job Summary

    Location: 15677 SW Oregon St, Sherwood, OR 97140
    Position: Full-time, salaried, exempt
    Compensation: Depending on experience (DOE)
    Schedule: 8:30 a.m.–5 p.m., with some flexibility, participation in a staffing on-call rotation, and shift coverage as needed
    Reports to: Director of Health Services

     

    What You’ll Do

    Coordinate resident care and clinical services

    • Partner with the Director of Health Services, Health Services Administrator, and other nurses to deliver quality, individualized resident care.
    • Assess residents as needed, addressing physical, emotional, social, and spiritual concerns.
    • Consult with residents, families, and outside providers to address care needs and support continuity of care.
    • Develop and maintain resident service plans at move-in, after 30 days, quarterly, and following significant changes in condition.
    • Coordinate quarterly care conferences with residents and families, document discussions, and ensure timely follow-through.
    • Support new resident move-ins by coordinating documentation, care tasks, supplies, and communication with the Sales and Health Services teams.
    • Maintain accurate resident records in PointClickCare (PCC), including service plans and point-of-care tasks.
    • Help coordinate medical appointments, transportation, provider documentation, and medical supplies as needed.
    • Assist with long-term care insurance paperwork as needed.

    Lead, train, and support the team

    • Supervise Assisted Living caregivers and medication technicians, providing ongoing coaching, feedback, and performance reviews.
    • Provide continuing education, staff training, and one-on-one coaching, including monthly medication technician meetings and training as needed.
    • Collaborate with staffing and staff development team members to support interviews, onboarding, orientation, and required training.
    • Help ensure employees complete required training and competency documentation before providing care.
    • Monitor attendance and address performance or behavioral concerns in partnership with leadership.
    • Participate in the staffing on-call rotation, arrange coverage for open shifts, and provide direct care when coverage cannot be secured.
    • Foster respectful communication and a culture of service excellence, teamwork, and accountability.

    Support quality, safety, and compliance

    • Review daily care reports, progress notes, and clinical alerts, and communicate concerns to the appropriate team members.
    • Review resident incident reports, investigate incidents promptly, coordinate follow-up interventions, and complete required documentation and reporting.
    • Review and address consulting pharmacy reports, coordinate pharmacy audits, and follow up on findings.
    • Ensure Health Services employees follow medication room procedures outlined in the Guide for Medication Assistants.
    • Stay current on Assisted Living infection control protocols and support their implementation.
    • Guide the Health Services team in following applicable federal and state regulations and company policies.
    • Participate in daily team meetings, weekly quality reviews, and audit preparation.

    What You Bring

    • An active Oregon Registered Nurse license.
    • Three to five years of relevant nursing experience, including experience in senior living and team leadership.
    • Strong knowledge of resident care in a senior living setting and a commitment to putting residents first.
    • Proven ability to lead, coach, and support a team through ongoing change.
    • Excellent communication skills and the ability to build relationships with residents, families, and coworkers.
    • High emotional intelligence, sound judgment, professional maturity, and a practical approach to problem-solving.
    • Strong organizational skills and the ability to prioritize, delegate, and manage multiple deadlines in a fast-paced environment.
    • Ability to understand care documentation, physician orders, and applicable policies and regulations.
    • Proficiency with email, electronic records, databases, word processing, and spreadsheets.
    • A proactive, dependable approach to routine responsibilities and unexpected challenges.
    • Willingness to participate in the staffing on-call rotation and work direct-care shifts when needed.

     

    Physical Requirements and Work Environment

    This position involves frequent standing and walking, including extended periods throughout the community; lifting up to 50 pounds; and pushing, pulling, bending, twisting, and reaching. The role requires manual dexterity for care tasks and handling supplies. Work may involve exposure to blood and bodily fluids, sharps, animal dander, and latex.
