There For You Home Care Services a Division of UniversaCare is hiring a full-time Registered Nurse Supervisor to lead clinical care for patients in their homes across the Waterbury, Hamden and surrounding Connecticut areas. The RN Supervisor oversees and supports our field nurses and home health aides, makes supervisory and assessment visits in patients’ homes, and makes sure every patient’s plan of care is followed safely and documented correctly.
Pay: $48.00 - $52.00 per hour
Work location: patients’ homes in Waterbury, Hamden and surrounding towns in Connecticut. Schedule: full-time, weekdays with on-call as assigned.
Responsibilities (include but are not limited to)
- Supervise, train and give clinical direction to RNs, LPNs and home health aides, including required supervisory visits in patients’ homes
- Conduct in-home patient assessments, including OASIS assessments at start of care, recertification, resumption of care, transfer and discharge, as assigned
- Develop, review and update individualized plans of care with physicians and the interdisciplinary team
- Review field staff visit notes and OASIS documentation for accuracy, timeliness and compliance
- Coordinate care with physicians, therapists (PT/OT/ST), social workers, families and community resources
- Handle clinical questions and changes in patient condition from field staff, and take part in the on-call rotation
- Support staff orientation, competency checks and in-service education
- Ensure compliance with Medicare/Medicaid Conditions of Participation, HIPAA and Connecticut regulations, and take part in quality improvement
Requirements
- Current, unrestricted Registered Nurse (RN) license in Connecticut
- At least 2 years of home health nursing experience; supervisory or charge experience preferred
- Experience completing and reviewing OASIS assessments
- Valid driver’s license, reliable transportation and current auto insurance (this role visits patients’ homes)
- Current BLS/CPR certification
- Strong leadership, communication and time-management skills
- Proficiency with electronic medical record (EMR) systems
