Matrix Home Care is seeking an experienced Registered Nurse (RN) to provide skilled nursing visits, including Start of Care (SOC), routine visits, follow -up visits, and patient assessments in the Loxahatchee, FL area.<\/span>
<\/p>
This is a field -based home care position ideal for an RN who is comfortable working independently, providing patient care in the home setting, and completing accurate and timely clinical documentation.<\/span>

Key Responsibilities:<\/span>
<\/p>
Provide skilled nursing care to patients in their homes<\/span>
<\/p>
Complete Start of Care (SOC), routine visits, follow -up visits, and assessments as assigned<\/span>
<\/p>
Perform patient evaluations and monitor changes in condition<\/span>
<\/p>
Provide patient and caregiver education<\/span>
<\/p>
Communicate updates and changes in patient status to the clinical team<\/span>
<\/p>
Complete all required documentation accurately and timely<\/span>
<\/p>
Follow physician orders, plan of care, and agency policies<\/span>
<\/p>
Maintain professionalism and compassionate care while working independently in the field<\/span>
<\/p>

Active Florida Registered Nurse (RN) license required<\/span>
<\/p>
Minimum of 1 year of nursing experience preferred<\/span>
<\/p>
Home health or home care experience strongly preferred<\/span>
<\/p>
Experience with Start of Care, skilled visits, assessments, and documentation preferred<\/span>
<\/p>
OASIS experience preferred, but not required<\/span>
<\/p>
Must be comfortable working independently in patients’ homes<\/span>
<\/p>
Strong clinical judgment, communication, and documentation skills required<\/span>
<\/p>
Must be dependable, professional, and patient -focused<\/span>
