Description

A Registered Nurse (RN) is needed for a PRN (as-needed) Home Care position in Cleveland, OH. Join a growing Home Care Company with flexible scheduling and competitive per-visit pay.

This is a PRN position covering SOC (Start of Care), ROC (Resumption of Care), Recerts, and Discharges. Pay is $50-$60 per visit (negotiable).

Responsibilities for this position include, but are not limited to:
  • Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge visits.
  • Perform comprehensive patient assessments and develop individualized care plans (OASIS).
  • Coordinate care with physicians, therapists, and other members of the care team.
  • Provide skilled nursing services and patient/family education.
  • Monitor patient progress and adjust the plan of care as needed.
  • Complete accurate, timely clinical documentation.

Requirements:
  • Current state license to practice as a Registered Nurse in Ohio.
  • Graduate of an accredited nursing program.
  • OASIS and home care experience preferred.
  • Current CPR certification.
  • Valid driver's license, reliable transportation, and auto insurance.
  • Strong assessment, communication, and time-management skills.
