Registered Nurse (RN) - PRN Home Health Visits | Arcadia, Alba, Sarasota, FL

Matrix Home Care is seeking experienced and compassionate Registered Nurses (RNs) for PRN home health visits in Arcadia, Alba, and Sarasota, FL. This position is ideal for nurses who enjoy providing one-on-one patient care in the home setting and are comfortable working independently while communicating closely with the interdisciplinary care team.

The Registered Nurse will complete skilled nursing visits, assess patient needs, provide education, coordinate care, and document services in accordance with agency, state, and federal home health requirements.

Responsibilities:

• Complete skilled nursing visits in the patient’s home
  
• Perform patient assessments and monitor changes in condition
  
• Provide wound care, medication education, disease management, and other skilled nursing services as ordered
  
• Review and follow physician orders and individualized care plans
  
• Educate patients and caregivers on medications, safety, disease management, and plan of care
  
• Communicate patient updates, changes in condition, and concerns to the care team and physician as appropriate
  
• Complete accurate and timely documentation in accordance with home health standards and regulatory requirements
  
• Maintain compliance with agency policies, AHCA requirements, and applicable Florida home health regulations
  

Requirements

• Active Florida Registered Nurse license
  
• Prior home health experience preferred
  
• OASIS/SOC experience preferred
  
• Current CPR/BLS certification preferred
  
• Valid driver’s license, reliable transportation, and auto insurance
  
• Strong assessment, documentation, time management, and communication skills
  
• Ability to work independently in the home setting
  
• Comfortable traveling within assigned service areas
