    This position is centered around ASAP consumer care, requiring strong knowledge of community-based services, compliance, and supervisory oversight.
    About the Role
    We are seeking an experienced RN or LPN for a full-time, field-based role. This position requires daily travel to client homes to complete supervisory visits, assessments, and ongoing care management.
    You will be working independently in the field while collaborating closely with case managers, families, and our internal team.
    Compensation & Benefits
    Salary: $90,000 – $105,000 annually (based on experience)
    Mileage reimbursement provided
    Full-time, stable Monday–Friday schedule
    Key Responsibilities
    Complete in-home supervisory visits for HHA/PCA caregivers
    Conduct client assessments and reassessments
    Ensure compliance with ASAP program requirements
    Communicate with ASAP case managers and families
    Monitor client conditions and report changes
    Maintain accurate and timely documentation
    Requirements 
    Active RN or LPN license in Massachusetts
    Minimum 1 year of home care experience preferred, but not required (new grads encouraged to apply)
    ASAP experience strongly preferred
    Must be comfortable working independently in the field
    Reliable transportation and willingness to travel daily
    Strong assessment and documentation skills
    Bilingual Spanish preferred, but not required

    Not a hospital or facility-based position
    Not remote or office-only
    Not a non-clinical/companion role
    Schedule
    Full-Time
