Per Diem Hospice Registered Nurse (RN) Intake/Admissions - Community Hospice

Why This Role Matters

As a Hospice Intake & Admissions RN, you are often the first point of contact on a patient's hospice journey. Your work sets the tone-providing comfort, clarity, and reassurance while guiding patients and families through the transition to hospice care.

What You'll Do

  • Serve as a key liaison between patients, families, providers, and our interdisciplinary team
  • Review and coordinate referrals to determine hospice eligibility in collaboration with physicians and care teams
  • Meet patients where they are-at home or in care facilities-to complete initial assessments
  • Initiate care plans, including ordering medications, medical equipment, and supportive services
  • Partner with social workers, chaplains, aides, therapists, and volunteers to provide holistic care
  • Ensure a smooth, supportive handoff to the Hospice Case Manager

Where You'll Work

This role offers a dynamic schedule, combining:

  • Time in our central intake office in Albany
  • Field visits to patients in homes, nursing homes, and assisted living communities across the region

What You Bring

  • Active New York State RN license
  • ASN required; BSN preferred
  • Home care experience (1+ year) strongly preferred
  • Hospice or end-of-life care experience is a plus
  • Valid driver's license and reliable transportation
  • Strong communication skills-you connect easily with patients, families, and clinical teams
  • Clear, concise documentation abilities
  • Comfort with computers and electronic health records
  • Excellent organization and time management skills

PER DIEM RATES based on Monthly Hours Commitments

Pay Range for non-per diem: $38.00-$50.18 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location
