A community-based behavioral health and developmental services is seeking a Registered Nurse (RN) in Brooklyn, NY to support adults with intellectual and developmental disabilities through nursing assessment, care coordination, crisis stabilization, and transitional planning.
About the Opportunity:
- Shift: Day shift
- Schedule: Monday to Friday
- Hours: 9am to 5pm
- Setting: Hybrid
Responsibilities:
- Conduct initial and ongoing nursing assessments for individuals served in the program
- Support admissions, intake evaluations, and transitional care planning
- Monitor health status, medication compliance, and therapeutic response
- Provide direct nursing interventions and coordinate follow-up care with providers and community resources
- Serve as a clinical liaison to families, program staff, and interdisciplinary team members while maintaining documentation and regulatory compliance
Qualifications:
- Minimum of 2 years of RN experience
- Active New York State RN license
- Experience supporting individuals with intellectual and developmental disabilities
- Strong clinical assessment and care coordination skills
- Excellent communication and documentation skills
- Ability to work independently
Desired Skills:
- Experience with behavioral health, community health, crisis care, care management, or transitional care
- Experience with OPWDD-related services
