What You’ll Do
- Provide skilled nursing care in patients’ homes
- Complete patient assessments and develop individualized plans of care
- Administer medications and treatments as ordered
- Monitor patient progress and changes in condition
- Educate patients and families on medications, disease management, and care needs
- Coordinate care with physicians, therapists, caregivers, and other members of the clinical team
- Complete accurate and timely clinical documentation
What You’ll Need
- Current, unrestricted RN license
- ADN or BSN from an accredited nursing program
- One year of home health experience required
- Current CPR certification
- Knowledge of Medicare and Medicaid guidelines
- Valid driver’s license and reliable transportation
- Strong clinical assessment, communication, and documentation skills
