Registered Nurse (RN) – Home Health – Per Visit / PRN

Penn Home Health Care is a Medicare-certified home health agency seeking experienced and compassionate Registered Nurses (RNs) to join our skilled home health team.

We are looking for clinicians who are committed to providing high-quality patient care while helping patients remain safely in their homes.

Competitive Per-Visit Rates

• SOC: $170 (+$30 incentive for same-day documentation completion)
• ROC: $120
• Recertification: $105
• Routine Skilled Visit: $85
• Discharge Visit: $110

Service Areas:
Philadelphia, Delaware, Chester, Montgomery, Bucks, and Berks Counties, Pennsylvania

Position Type:
Per Visit / PRN

Responsibilities:
• Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, Supervisory, Routine Skilled Nursing, and Discharge visits.
• Perform comprehensive patient assessments.
• Develop and implement individualized plans of care in collaboration with physicians.
• Provide skilled nursing services including disease management, medication management, wound care, patient education, and care coordination.
• Complete OASIS assessments as assigned.
• Communicate significant findings and changes in condition to physicians and interdisciplinary team members.
• Participate in case conferences and quality improvement activities.
• Maintain accurate and timely electronic documentation.

Qualifications:
• Current Pennsylvania Registered Nurse license in good standing.
• Current CPR certification.
• Valid driver's license and reliable transportation.
• Minimum one year nursing experience preferred.
• Home health experience preferred.
• OASIS experience preferred but willing to train qualified candidates.
