We are seeking a skilled Registered Nurse (RN) to provide home health services across multiple counties in southern West Virginia. This role focuses on delivering high‑quality clinical care, completing comprehensive assessments, and ensuring accurate documentation while working independently in the field.
Develop, implement, and revise individualized plans of care in collaboration with physicians and interdisciplinary team members.
Perform comprehensive nursing evaluations, including OASIS assessments and medication reconciliation.
Supervise home health aides and LPNs in accordance with state and federal guidelines.
Conduct required supervisory visits and ensure patients meet home health eligibility and medical necessity criteria.
Deliver evidence‑based nursing care aligned with infection control standards and the established plan of care.
Document all patient visits accurately and in compliance with policy and payer requirements.
Sync and submit documentation within required timeframes.
Communicate effectively with physicians, patients, families, and internal staff regarding care plans, orders, scheduling, and patient status.
Support quality initiatives, survey readiness, and performance improvement efforts.
Work proactively to reduce avoidable hospitalizations and promote positive patient outcomes.
Collaborate with leadership and complete assigned tasks and required learning.
Assist with onboarding and precepting new staff or students.
Participate in on‑call and weekend rotations as needed.
Follow utilization management processes and agency best practices.
Active, unrestricted Registered Nurse (RN) license in the state of practice.
Valid driver’s license, auto insurance, and reliable transportation.
Current CPR certification (or ability to obtain within 90 days).
Ability to provide care in diverse home environments.
1+ year of home health experience.
Strong communication, documentation, and organizational skills.
Ability to work independently in the field.
PICC and Port‑A‑Cath care.
Indwelling and suprapubic catheter care.
Venipuncture and lab specimen collection.
Comprehensive wound care, including wound vac therapy.
Tracheostomy care and patient/caregiver education.
Ostomy care and troubleshooting.
Strong infection prevention practices.
Mileage Reimbursement: Yes (from first patient to last patient).
Dress Code: Navy Blue scrubs.
On‑Call Requirements: 2–3 weekday on‑call shifts per week; weekend on‑call up to every other weekend.
