Conduct in-home nursing assessments and reassessments
Develop and update individualized client care plans
Supervise Home Health Aides, CNAs, and caregivers
Ensure caregivers are following established care plans
Monitor caregiver performance and compliance
Provide caregiver education, training, and re-education as needed
Communicate with clients, families, and office staff
Complete accurate and timely nursing documentation
Help maintain high standards of quality care and client safety
Active Massachusetts Registered Nurse (RN) license
Strong assessment and clinical judgment skills
Excellent communication and organizational abilities
Ability to work independently
Reliable transportation
Home care, home health, geriatric, or supervisory nursing experience preferred
