    Lead RN / Home Care Clinical Supervisor

    Part-Time | 20–30 Hours Per Week

    Azalea Homecare is seeking a Registered Nurse (RN) to serve as our Lead RN / Home Care Clinical Supervisor.

    This is a growth-oriented clinical leadership position with the opportunity to expand in responsibility as Azalea Homecare grows. The role focuses on assessments, care planning, nursing oversight, caregiver support, compliance, documentation, and care coordination.

    What You Will Do

    • Complete client acuity assessments within two business days of referral notification.
    • Perform initial nursing assessments and home safety reviews.
    • Develop, review, sign, and update individualized care plans.
    • Identify fall risks, transfer needs, caregiver skill requirements, and changes in condition.
    • Conduct supervisory visits and reassessments.
    • Communicate clinical concerns and care-plan changes with families, caregivers, case managers, and internal teams.
    • Support clinical requirements for CCSP, SOURCE, Structured Family Caregiving, Private Pay, and Long-Term Care Insurance clients.
    • Address PSS/PSSX reviews and clinical documentation requests.
    • Provide caregiver education, competency evaluations, and proxy caregiver training when applicable.
    • Provide RN-level oversight of nursing personnel, including LPNs, and review/sign clinical documentation when required.
    • Conduct quarterly client clinical file audits and support review of nursing credentials and competencies.
    • Assist Billing and Operations with clinical documentation issues affecting authorization or billing readiness.
    • Track assessments, supervisory visits, reassessments, care-plan updates, and outstanding clinical documentation.
    • Serve as a primary clinical resource for the agency.
    Required Qualifications

    • Active, unrestricted Registered Nurse (RN) license authorizing practice in Georgia.
    • Graduate of an accredited nursing program.
    • Current CPR/BLS certification.
    • Strong clinical judgment, communication, organization, and documentation skills.
    • Ability to work independently and collaborate with an interdisciplinary team.
    • Comfortable using electronic documentation systems and Microsoft Office.
    • Reliable transportation and ability to travel to client homes.
    • Availability for 20–30 hours per week.
    Preferred Qualifications — Not Required

    Experience in one or more of the following is preferred but not required:

    • Two or more years of RN experience.
    • Home care, home health, geriatrics, community-based nursing, or clinical supervision.
    • Healthcare compliance, quality assurance, or clinical documentation auditing.
    • Medicaid policies, documentation requirements, or service authorizations.
    • Georgia Medicaid waiver programs, including CCSP, SOURCE, or Structured Family Caregiving.
    • Case management or care coordination.
    • Caregiver education and competency assessment.
    • WellSky or similar home-care software.

    Schedule

    This is a part-time position averaging 20–30 hours per week. Responsibilities may include client visits, assessments, supervisory visits, office responsibilities, documentation, care planning, case-manager communication, staff training, and clinical audits. Hours may vary based on client needs and required nursing visits.
