Heart & Vascular Navigator (Heart Failure, AMI, & CT Surgery) 

patients and families navigating Heart Failure, Acute Myocardial Infarction (AMI), and Cardiothoracic Surgery care. This role coordinates care, provides education, and supports patients throughout their healthcare journey to ensure exceptional outcomes and experiences.

What You'll Do

  • Guide and support Heart Failure, AMI, and CT Surgery patients throughout their care journey.
  • Assess patient and family needs and develop individualized care plans.
  • Educate patients and caregivers on diagnoses, treatment plans, medications, and recovery expectations.
  • Coordinate care and facilitate communication among patients, families, physicians, and interdisciplinary teams.
  • Support smooth transitions of care, including discharge planning and post-hospital follow-up.
  • Promote patient and family-centered care through collaboration, shared decision-making, and advocacy.

Participate in quality improvement initiatives that enhance patient outcomes and the overall care experience
