Registered Nurse (RN) – Part-Time Group Home
Bronx, NY | Part-Time / Flexible Schedules

Bond Health is seeking Registered Nurses (RNs) for multiple part-time/per-diem opportunities at group home residences in the Bronx. The RN will evaluate and monitor the health and safety needs of individuals with developmental disabilities, promote wellness, and ensure nursing care is delivered in compliance with applicable federal and state regulations and established policies

    Essential Responsibilities
    • Coordinate medical care for assigned individuals; monitor and assess health needs and arrange follow-up care as needed.
    • Assess health status through direct observation, including signs and symptoms of discomfort, distress, or illness; evaluate physical status changes and responses to care and treatment; document and follow up on findings.
    • Communicate with physicians, families, interdisciplinary team members, and clinicians involved in providing services.
    • Communicate and follow up on hospitalizations, emergency room visits, injuries, trauma, accidents, illness, distress, and other health concerns as required.
    • Accompany individuals to medical appointments when necessary to advocate for and discuss health issues and communicate findings to team members.
    • Respond, when necessary, to urgent or emergent calls outside routine working hours regarding individuals assigned to the caseload.
    • Attend meetings with management, staff, families, and clinicians regarding health-related issues.
    • Develop individual-specific nursing plans and protocols and update them as an individual's status changes.
    • Supervise AMAP administration of prescribed medications and routine treatments in accordance with federal and state regulations and established policy.
    • Administer STAT medications and treatments as prescribed; participate in in-services and train AMAPs as needed; complete AMAP certification, recertification, and decertification processes as necessary.
    • Perform other duties as assigned.
    Qualifications
    • Must be currently licensed as a Registered Professional Nurse (RN) in New York State.
    • CPR certification required.
    • Experience working with individuals with developmental disabilities preferred.
    • Must demonstrate excellent written and oral communication skills.
    • Ability to handle multiple priorities and work as a member of a team.
    • A valid NY State or Tri-State Driver's License for more than one year, with a good driving record, is preferred.
