ChristianaCare HomeHealth is seeking a compassionate and skilled Registered Nurse (RN) to join our dedicated team caring for primarily geriatric patients throughout the community. Position Highlights
Full-time, day shift schedule
One weekend per month
Two holidays per year
Limited on-call responsibilities
Care for patients throughout the community
Autonomy to manage your daily schedule while supported by an interdisciplinary team
Primary Responsibilities
Provide skilled nursing care and prescribed treatments to predominantly geriatric patients in their homes in and around North Wilmington, DE
Perform comprehensive patient assessments and ongoing evaluations
Develop, implement, and update individualized care plans
Administer medications, treatments, and IV therapy as ordered
Educate patients and caregivers regarding disease management, medications, safety, and wellness
Coordinate care with physicians, therapists, social workers, and other healthcare professionals
Facilitate discharge planning and transitions of care
Qualifications
Current RN licensure in Delaware or eligibility through a Compact State License
BSN preferred; ADN candidates may be considered with a commitment to obtaining a BSN within the designated timeframe
Minimum of one year of RN experience
Home Health, Medical-Surgical, Geriatric, or Community Health experience preferred
IV therapy and wound care experience preferred or willingness to complete training
Current valid driver's license and reliable transportation

Hourly Pay Range: $36.10 - $57.76
This pay rate/range represents ChristianaCare’s good faith and reasonable estimate of compensation at the time of posting.
