What You'll Do

• Complete patient admissions and assessments
• Provide skilled nursing care in the home setting
• Administer medications and perform treatments
• Address urgent patient needs, including catheter care and fall follow-up
• Develop and update individualized care plans
• Educate patients and families on health and disease management
• Coordinate care with physicians and other healthcare providers
• Document care and patient progress accurately

What You'll Need

• Current Registered Nurse (RN) license in Utah
• Home health experience preferred
• Knowledge of infection control and nursing practices
• Strong assessment and clinical skills
• Excellent communication and documentation skills
• Reliable transportation, valid driver's license, and auto insurance
• Ability to work independently in the field
• Ability to lift, transfer, and assist patients as needed
