Schedule: Per Diem- As Needed

SUMMARY:

As a Community Health Nurse, you will focus on providing nursing interventions, patient education, and care management to a variety of clients in our community. This promotes good health and one on one patient care in the comfort of their own home.

RESPONSIBILITIES:

  • Patient Care & Service - Provide comprehensive assessments of the bio-psycho-social needs of acutely ill clients in their home; perform skilled nursing care and prescribed treatments to clients based on MD orders; provide client and family education in accordance with client assessment and plan of care.

  • Planning - Develop and document individualized care plans customized for each patient’s unique needs, with support from the interdisciplinary health team as needed; maintain effective communication to convey patient health status, treatment plans and progress.

  • Communication -Maintain effective communication to convey patient health status, treatment plans and progress; communicate with office and supervisory staff regularly; coordinates client services and/or referrals in an efficient and timely manner.

  • Documentation - Accurately and completely document client care; document and implement physician orders per D.O.H. and program standards.

  • Compliance - Adhere to required department and system protocols, regulations (local, state, federal) and education requirements.

REQUIRED QUALIFICATIONS:

  • Diploma or Associate’s Degree in Nursing.

  • Registered Nurse licensure in New York State.

  • Valid NYS Driver’s License.

PREFERRED QUALIFICATIONS:

  • Two (2) years experience in home care preferred

  • Compassionate, independent ,and patient focused

  • Comprehensive nursing assessments and documentation, decisive judgment and professional behavior

  • Bilingual preferred

EDUCATION:

AS: Nursing (Required)

LICENSES / CERTIFICATIONS: 

BLS - Basic Life Support- American Heart Association (AHA), RN- Registered Nurse- NYS Office of Professions

PHYSICAL REQUIREMENTS:

Drive- Clinical- Roles in this category require frequent driving combined with mobile clinical care across settings such as emergency response, mobile clinics, and home health environments. Staff must repeatedly enter and exit vehicles, occasionally lift and carry equipment weighing up to 100 lbs. or more, and perform care within patients’ homes or other confined, unpredictable, or uncontrolled environments. These positions demand strong fine and gross motor control, firm grasping, and heightened visual and auditory awareness to ensure safe and effective care delivery. Emotional and cognitive demands are substantial due to rapid decision‑making, direct exposure to critical or sensitive situations, and independently managing patient care in the field or home setting. Environmental exposures may include traffic hazards, infectious agents, extreme temperatures, variable home conditions, and hazardous materials. Valid licensure, physical readiness, and flexibility for extended or on‑call schedules are typically required.

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components.
