Competitive salary: $90,000-$105,000, based on experience.

About the Role
We're seeking an experienced Registered Nurse Care Coordinator to support our Collaborative Care and Chronic Disease Management programs.
In this role, you'll serve as a patient advocate and clinical resource, partnering with providers and interdisciplinary teams to coordinate care, improve outcomes, and empower patients to successfully manage chronic conditions.
Schedule:
Monday - Wednesday | 9:00 AM - 6:00 PM
Thursday & Friday | 8:00 AM - 5:00 PM
What You'll Do
Coordinate care and advocate for patients with chronic conditions
Assess patients and develop individualized care plans
Educate and coach patients to improve self-management
Lead interdisciplinary care team meetings
Collaborate with providers and clinical teams to improve outcomes
Identify high-risk patients using population health tools
Document patient care plans and maintain accurate EMR documentation
Support referrals, care transitions, and quality improvement initiatives
What You'll Bring
Required
Bachelor's degree in Nursing
Active RN license in good standing (multistate preferred)
Current CPR certification
Five or more years of nursing experience
Preferred
Care coordination, case management, chronic disease management, or population health experience
Motivational interviewing experience
Strong communication, organization, and critical thinking skills
Experience with EMRs and Microsoft Office
