A healthcare organization is seeking a Registered Nurse (RN) Case Manager to coordinate patient care and discharge planning services in an acute care setting. This role is responsible for advocating for patients, facilitating care transitions, and collaborating with interdisciplinary teams to achieve quality, cost-effective outcomes.
Assess patient needs and develop individualized care coordination and discharge plans
Coordinate referrals and connect patients with appropriate community and healthcare resources
Monitor patient progress and advocate for services that support optimal outcomes
Manage assigned caseloads based on patient needs, strengths, barriers, and preferences
Collaborate with physicians, nurses, social workers, and interdisciplinary teams to coordinate patient care
Facilitate safe and efficient transitions of care and discharge planning activities
Maintain accurate and timely documentation of patient assessments, interventions, and outcomes
Promote quality, cost-effective care through effective resource management
Adapt to changing priorities in a fast-paced healthcare environment
Demonstrate professional accountability and maintain patient-centered care standards
Graduate of an accredited School of Nursing
Current Registered Nurse (RN) license in the state of practice
Associate Degree in Nursing required; Bachelor of Science in Nursing (BSN) preferred
Minimum of two years of Registered Nurse experience in an acute care setting
Previous case management experience preferred
Case management certification preferred
Strong assessment, care coordination, communication, and critical thinking skills
Ability to manage multiple priorities and work effectively within an interdisciplinary team
