    Interim HealthCare is seeking an experienced and compassionate Registered Nurse (RN) Case Manager to join our team in Portsmouth, New Hampshire.

    Position Details

    • Location: Portsmouth, NH and surrounding communities
    • Schedule: Monday-Friday
    • Employment Type: Full-Time

    What You'll Do

    As an RN Case Manager, you'll take ownership of a dedicated patient caseload while providing high-quality skilled nursing care in the home.

    Your responsibilities will include:

    • Perform comprehensive nursing assessments and develop individualized plans of care.
    • Provide skilled nursing visits and ongoing case management.
    • Monitor patient progress and identify changes in condition.
    • Coordinate care with physicians, therapists, caregivers, and other healthcare professionals.
    • Educate patients and families on disease management, medications, treatments, and strategies for maintaining independence.
    • Provide compassionate support to patients and families during important moments in their healthcare journey.
    • Complete accurate and timely clinical documentation.
    • Supervise and collaborate with members of the interdisciplinary care team.
    • Advocate for patients and ensure continuity, quality, and safety of care.
    • Help patients achieve their goals and remain safely in the place they call home.

    What We're Looking For

    • Current Registered Nurse (RN) license in the State of New Hampshire
    • Minimum of two (2) years of skilled nursing experience preferred
    • Home health experience preferred, but we are willing to consider strong candidates with relevant clinical experience
    • Previous nursing supervisory experience preferred
    • Recent wound care experience preferred
    • Current CPR Certification
    • Valid driver's license and reliable transportation
    • Strong clinical assessment, communication, organizational, and critical-thinking skills
    • Ability to work independently while collaborating effectively with a multidisciplinary team
    • Compassionate, patient-centered approach
    • Genuine desire to make a difference in the lives of patients and families
