• Become a Health Detective: As an RN Case Manager, youll conduct in-depth assessments, uncovering the unique needs of each client.
• Be a Master Planner: Youll use your clinical expertise to create personalized care plans that arent just about treatment-theyre about helping people live their best lives.
• Make a Real Impact: Youll see the direct results of your work as you help clients achieve their goals, whether thats safely managing their medications or simply staying independent at home.

What Were Looking For

Youre a great fit if youre a compassionate RN whos ready to bring your passion and skills to a rewarding home care environment.

RN Case Manager means you also meet the following requirements:

• Youre a graduate of an accredited nursing program.
• You have a current Illinois Registered Nurse (RN) license.
• Youve got a minimum of one year of experience as an RN.
• Youre CPR-certified and a pro at safety and infection control.

Compensation: $45.00 - $50.00 per hour
