Position Title: Registered Nurse (RN) – Case Manager
Location: Burbank, CA 91505
Duration: 13 Weeks
Work Schedule: Days | 8AM–4:30 PM | 40 hrs/weekly
Pay rate: $60-$70/hr

Responsibilities for this position include, but are not limited to:

• Job Overview:
  Provide acute-care case management services, including care coordination, utilization review, and discharge planning for adult and geriatric patients.
  Responsibilities:
• Assess patients and coordinate appropriate care and services.
• Develop discharge plans and coordinate transitions of care.
• Determine medical necessity using evidence-based guidelines.
• Coordinate with physicians, healthcare teams, HMOs, and IPAs.
• Maintain accurate documentation using Epic.
• Support utilization review and regulatory compliance.
• 
  Requirements:
• Minimum 2 years of acute-care Case Management experience involving care coordination and discharge planning.
• Active RN license; pending license accepted if issued before the start date.
• Epic experience required.
• Previous travel experience preferred; first-time travelers may be considered after screening.
• Experience with acute-care settings, including ICU, Medical-Surgical, or Step-Down/PCU.
• Must work every other weekend as scheduled and commit to working 3 of the 5 designated holidays.
