Registered Nurse (RN) Care Manager - Hospice

Ohio's Hospice is seeking a compassionate and experienced Registered Nurse (RN) Care Manager to provide high-quality hospice care to patients and families throughout Licking, Muskingum, and portions of Knox County, including the Mount Vernon and Centerburg areas.
This is a field-based nursing position serving patients in both private homes and care facilities. The RN Care Manager will oversee a caseload of approximately 25-30 patients and will play an important role in coordinating care, managing symptoms, supporting families, and helping patients experience comfort and dignity throughout their hospice journey.

Position: Registered Nurse (RN) Care Manager
Employment: Full-time
Schedule: Monday-Friday, 8:00 AM-4:30 PM
Caseload: Approximately 25-30 patients
Service Area: Licking, Muskingum, and portions of Knox County
Areas Served: Includes Mount Vernon and Centerburg
Patient Setting: Patient homes and healthcare facilities
Weekend Requirement: Rotating weekends, approximately every 8th weekend
Holiday Requirement: Participation in a holiday rotation
Travel: Regular travel throughout the assigned service area is required
About the Role
As an RN Care Manager, you will serve as a primary clinical resource for your patients and their families. You will use your nursing expertise to assess patient needs, coordinate individualized care, manage symptoms, and communicate closely with the hospice interdisciplinary team.
You will work with physicians, social workers, hospice aides, chaplains, and other care team members to make sure each patient's needs are understood and addressed.
This position offers the opportunity to build meaningful relationships with patients and families while providing skilled nursing care focused on comfort, quality of life, and the goals of hospice care.
What You'll Do
Manage an assigned caseload of approximately 25-30 hospice patients.
Provide skilled nursing care to patients in their homes and in healthcare facilities.
Complete comprehensive nursing assessments and develop individualized plans of care.
Monitor patient conditions and recognize changes that may require adjustments to the plan of care.
Assess and manage pain and other symptoms to promote comfort and quality of life.
Coordinate care and communicate effectively with physicians and members of the interdisciplinary team.
Educate patients, families, and caregivers regarding disease progression, medications, symptom management, and available hospice services.
Provide emotional support and serve as an advocate for patients and families.
Communicate changes in patient condition and care needs to the appropriate members of the care team.
Maintain accurate and timely documentation of assessments, interventions, medications, communications, and patient outcomes.
Coordinate services and resources to support patients across home and facility settings.
Follow applicable nursing standards, hospice regulations, organizational policies, and the established plan of care.
Participate in weekend and holiday rotations as scheduled.
Travel throughout the assigned service area to provide patient care.
What We're Looking For
Required Qualifications
Graduate of an accredited nursing program.
Current Registered Nurse (RN) license in the State of Ohio, in good standing.
Current CPR certification.
Valid driver's license with a safe driving record.
Reliable transportation and ability to travel throughout the assigned service area.
Ability to safely drive in a variety of weather and road conditions.
Ability to meet state-required automobile insurance requirements.
Basic computer skills and ability to accurately document patient care.
Successful completion of required pre-employment screening, including background check and drug screening.
Preferred Qualifications
Two years of acute care nursing experience.
Experience in hospice, palliative care, home health, community nursing, or a related setting.
Certified Hospice and Palliative Nurse (CHPN) certification.
CHPN certification support and preparation are available for eligible employees.
