Matrix Home Care is currently seeking experienced and compassionate Registered Nurses (RN) and Licensed Practical Nurses (LPN) to provide Start of Care (SOC) assessments and skilled nursing visits throughout Brevard County, Florida.<\/span>

Responsibilities:<\/span>
<\/div>
• Complete Start of Care (SOC), recertification, resumption of care, and routine skilled nursing visits<\/span>
<\/div>
• Perform comprehensive patient assessments and documentation<\/span>
<\/div>
• Develop and implement individualized care plans<\/span>
<\/div>
• Educate patients and families regarding medications, treatments, and disease management<\/span>
<\/div>
• Maintain accurate and timely Medicare documentation<\/span>
<\/div>
• Collaborate with physicians and interdisciplinary healthcare teams<\/span>
<\/div>
• Ensure compliance with company policies and home health regulations<\/span>
<\/div>

Requirements<\/h3>

RN Qualifications:<\/span>
<\/div>
• Active Florida RN license<\/span>
<\/div>
• Home health experience preferred<\/span>
<\/div>
• OASIS documentation experience strongly preferred<\/span>
<\/div>
• KanTime EMR experience preferred<\/span>
<\/div>
• Valid driver’s license, reliable transportation, and auto insurance required<\/span>
<\/div>

LPN Qualifications:<\/span>
<\/div>
• Active Florida LPN license<\/span>
<\/div>
• Home health and skilled nursing visit experience preferred<\/span>
