    Description

    We are seeking a dedicated and skilled professional to join our team. This role requires a commitment to providing excellent healthcare services to our clients in Rhode Island. The successful candidate will play a critical role in health maintenance, disease prevention, and case management.

    Shift Requirements

    • Monday to Friday: 8 AM - 5 PM
    • One weekend required: 8 AM - 5 PM

    Key Responsibilities

    • Assess the health of individuals and implement care plans.
    • Supervise care plans and staff members.
    • Observe, assess, plan, implement, and evaluate nursing support for various health conditions in community or small facility settings.
    • Collaborate with other health professionals to plan, implement, and evaluate health programs and services.
    • Coordinate communications and actions related to health and safety supports.
    • Manage the ordering, reviewing, monitoring, and disposal of medications as prescribed.
    • Ensure policies on medication administration and safety are in place and reviewed annually.
    • Report progress to physicians when clients' conditions change.
    • Transcribe or ensure transcription of physician phone orders and medically related reports.
    • Complete required nursing documentation.
    • Maintain routine documentation of health status in nurses' notes.
    • Ensure maintenance of furnishings, equipment, and supplies to prevent disease spread.
    • Direct and supervise licensed practical/vocational nurses as needed.
    • Schedule and complete health-related appointments, ensuring timely responses to findings.
    • Communicate with inpatient nursing and discharge coordinators during hospitalizations.
    • Provide new employee training and annual recertification on health and safety topics.
    • Train unlicensed staff on person-specific health maintenance, safety, and disease prevention needs.
