Wed: 8a-8p
Thursday: 8-4p

Job Summary:
• This position involves providing primary care services in a multi-specialty setting. 
• The role requires a Registered Nurse with experience in primary care to work onsite in Clove Road, NY. 
• The job is a contract position, requiring availability Monday through Friday for 40 hours a week.

Requirements:
• Develop care plans based on patient needs and nursing team skills.
• Implement organizational programs and procedures.
• Collect and analyze data to adjust processes as needed.
• Maintain required licensure and engage in ongoing education.
• Assist patients with self-care management.
• Follow up on abnormal lab results and manage medication refills.
• Coordinate outpatient work-ups and treatment plans when necessary.

Qualifications:
• Completion of nursing training from an accredited school of nursing.
• Minimum of one to two years' experience in Medical Practice/Primary Care, Med Surg, ER, or Occupational Health.
• Current New York State Registered Nurse license.
