    To call discharged Emergency Department patients at home to follow up on their care.

    Responsibilities:
    • 1. Responsible for using EPIC reports to identify ED patients that were discharged .
    • 2. Responsible for calling discharged patients to follow up on their visit and provide any necessary clarification of discharge instructions.
    • 3. Provides service recovery as needed.
    • 4. Calls to be made within 48 hours of patient discharge.
    Qualifications:

    EDUCATION

     

    Graduate of an accredited school of nursing (Diploma, AD, BS, BSN)

     

    EXPERIENCE

     

    2 years ED experience

     

    LICENSURE

     

    RN In State of CT or Permit; Basic Life Support (BLS)

     

    SPECIAL SKILLS

     

    Computer skills, strong service excellence telephone skills

     

    PHYSICAL DEMAND

     

    May be exposed to infectious diseases. Moderate stress. Light Duty.

     

    Additional Information:

    2 years of ED experience required.

    This is a non-clinical role. This person calls backs 100 percent of all discharged patients from the ED the day prior. They assist with any questions the patients may have about ongoing symptoms, help to facilitate follow up care or appointments, and often help with troubleshooting issues like prescriptions.
