Job Opportunity in Castro Valley, CA
Join our team as a professional Registered Nurse (RN) in a Certified Comprehensive Stroke Center. We are seeking a dedicated individual to provide guidance, education, coordination, and support to stroke patients, families, and caregivers across the continuum of care.
- License/Certifications Required: NIHSS, CA RN, BLS-Basic Life Support
- Preferred Certifications: Certified Neuroscience Registered Nurse (CNRN) or Stroke Certified Registered Nurse (SCRN)
- Schedule: M-F, 8-1630, with occasional weekend community events
- Experience: 5 years recent acute care experience with stroke patients or as a nurse navigator
- Skills: Effective communication, chart review, medical terminology, Microsoft Office proficiency
Responsibilities include:
- Providing comprehensive inpatient stroke education
- Reinforcing stroke prevention strategies and medication adherence
- Leading community outreach initiatives
- Participating in stroke program initiatives and quality improvement activities
- Collaborating to improve transitions of care and support evidence-based stroke management
