Description

A health services organization is seeking a licensed Registered Nurse (RN) in the Bronx, NY. This role supports clinical care, care coordination, and health education for individuals served in a clinic-based setting.

About the Opportunity:

  • Shift: Day shift
  • Schedule: Monday to Friday
  • Hours: 9am to 4:30pm
  • Setting: Clinic-based

Responsibilities:

  • Coordinate and follow through on health care services as part of an interdisciplinary team
  • Arrange medical services, make referrals, and provide follow-up on medical concerns
  • Support care management needs through assessment, documentation, and monitoring of treatment plans
  • Provide medication administration oversight, first aid, and emergency response as needed
  • Conduct staff training and maintain compliance with infection control and regulatory requirements

Qualifications:

  • At least 2 years of experience as a Registered Nurse in community-based health services
  • Graduate of an accredited professional nursing program with a BSN
  • Current New York State Registered Nurse license and registration
  • CPR certification

Desired Skills:

  • Experience working with individuals with intellectual and developmental disabilities
  • Knowledge of adaptive equipment and related support needs
  • Strong organizational, communication, and interpersonal skills
  • Experience with care management and clinical documentation
