Job description 
 
We are looking for a Registered Nurse to care for our patients and facilitate their speedy recovery in South San Antonio. You will be responsible for educating them and their families on preventions and healthy habits. The ideal candidate will be a responsible and well trained professional able to give the best nursing care with little supervision. Your primary functions are to administer skilled nursing care for clients of all ages in their place of residence, coordinate care with the interdisciplinary team, patient/family and referring agency; follow health and safety guidelines; and assume the responsibility for coordination of care. The goal is to promote patient's well-being by providing high quality nursing care.
 
Education
Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.

Licensure
Current Texas State license as a Registered Nurse, and a current Texas Driver's License. Proof of CPR.

Experience
Two years' experience as a Registered Nurse in a clinical care setting preferred, home health experience preferred. Effective communication to patients/family, team members, and other health care professions. Good interpersonal skills. Experience with Kinnser/Wellsky is a plus.
Adult skilled Home Health experience required. Bilingual Required. 

Skills 
Assess home health patients to identify the physical, psychosocial, and environmental needs as evidenced by documentation, clinical record, case conference, team report, and evaluations. Implement/develop/document the plan of care to ensure quality and continuity of care. Provide care utilizing infection control measures that protect both the staff and the patient (OSHA). Supervise and provide clinical direction to HHA/LVN providing services. Assure continuity of quality patient care delivered with appropriate documentation. Monitor assigned cases to ensure compliance with requirements of third-party payor. Demonstrate commitment, professional growth, and competency. Promote Agency philosophy and administrative policies. Perform on-call responsibilities and provide on-call service to patients/families as assigned.
