Position Overview:

The Registered Nurse - Long Term Services and Supports (LTSS) Screener performs the mandated service of long-term services and supports to conduct pre-admission screening and medical need determination for Medicaid-funded nursing home placement and community-based services. This position maintains a collaborative working relationship with the Department of Social Services (DSS) to conduct joint home visits to assess/screen individuals for Medicaid-funded services; pediatric LTSS screenings are completed independent of DSS. The RN completes a nursing assessment and completion of the Universal Assessment Instrument (UAI), a tool used to determine medical eligibility of vulnerable members of our population for Medicaid-funded LTSS. The successful applicant works independently, is organized, and communicates effectively with patients, staff, community groups, private providers, and district management. The LTSS Nurse is an integral member of the healthcare team, contributing to a safe, supportive, and clinically excellent environment for vulnerable populations in the community setting.

Key Responsibilities

A. LTSS Screening & Assessment

• Physically travel to participants' homes to complete LTSS screenings throughout the Rappahannock Rapidan Health District (Culpeper, Fauquier, Madison, Orange, and Rappahannock counties).
• Complete face-to-face comprehensive assessments of participants to determine appropriate level of services and supports per regulated guidelines and timelines.
• Assess for medical necessity and authorize appropriate waiver services.
• Complete the Universal Assessment Instrument (UAI) to determine medical eligibility for Medicaid-funded LTSS.
• Conduct pre-admission screening for Medicaid-funded nursing home placement and community-based services.
• Complete pediatric LTSS screenings independent of DSS.

B. Collaboration & Communication

• Maintain a collaborative working relationship with the Department of Social Services (DSS) to conduct joint home visits.
• Communicate effectively with patients, families, staff, community groups, private providers, and district management.
• Document all client encounters per documentation standards.
• Participate in appeals for denied waiver services.

C. Compliance & Documentation

• Document all client encounters accurately, completely, and in a timely manner in accordance with regulatory and organizational standards.
• Maintain working knowledge of home and community-based services.
• Ensure compliance with applicable federal, state, and organizational policies, regulations, and clinical standards.
• Protect confidential and sensitive participant information in accordance with HIPAA and applicable privacy standards.
• Successfully complete LTSS certification course.

D. Performance & Administrative Responsibilities

• Meet or exceed all productivity standards and metrics benchmarks.
• Demonstrate excellent punctuality and attendance during work hours.
• Maintain professional competency through continuing education and participation in professional development activities.
• Complete other duties as assigned within the scope of licensure and organizational needs.

Qualifications & Requirements

• Education: Associate's Degree in Nursing (ADN) required. Bachelor of Science in Nursing (BSN) preferred.
• Licensure: Unrestricted Registered Nurse (RN) license in the state of Virginia required.
• Certifications: BLS-CPR Certification required. LTSS certification (must successfully complete course upon hire).
• Experience: Minimum one year of nursing experience required. One to three years of case management experience preferred. Experience in home visiting or home health preferred. Knowledge and experience in LTSS screening preferred.
• Skills: Strong assessment skills with the ability to evaluate medical necessity and determine appropriate level of care. Excellent time management skills with the ability to work independently. Excellent computer skills and attention to detail. Strong interpersonal and communication skills with the ability to interact effectively with patients, families, providers, and DSS partners. Must be comfortable traveling throughout the district to complete screenings in conjunction with a social worker from DSS.
• Transportation: Must have own transportation. Mileage will be reimbursed.

Working Conditions

• Field position requiring physical travel to participants' homes throughout the Rappahannock Rapidan Health District (Culpeper, Fauquier, Madison, Orange, and Rappahannock counties).
• Conduct joint home visits with Department of Social Services (DSS) social workers.
• Communicate and interact with patients, families, providers, DSS partners, and district management on a daily basis.
• Prepare for the possibility of unexpected or sensitive situations during home visits.
• Work independently with minimal direct supervision.
• Mileage reimbursement provided for travel.

Physical Requirements:

Must be able to lift and carry up to 15 pounds. Position requires standing between 33% and 66% of time on duty, walking between 33% and 66% of time on duty, and sitting less than 33% of time on duty. Use of hands to finger, handle, or feel over 66% of time on duty. Reaching with hands or arms less than 33% of time. Full ability to talk and hear. Correctable vision required. Must be able to travel to patient homes throughout the district. Must be able to enter and navigate various home environments.

Schedule:

Monday through Friday, 8:00 AM - 4:30 PM
