Coordinate interdisciplinary patient care: Serve as the central point of contact across clinical disciplines to build and execute seamless care plans that keep the patient's recovery on track.
Track and communicate patient progress: Monitor health status, maintain precise medical records, and promptly inform the care team and family of critical changes to ensure continuous, high-quality care.
Execute physician and nursing orders: Carry out medical directives efficiently and accurately to ensure timely treatments and maintain high standards of patient safety.
Lead seamless discharge planning: Assess each patient's post-care needs early and collaborate with the healthcare team to prepare safe, supportive transitions back home or to follow-up care.
Licensure/Certification/Registration:
BLS Provider credentialed from the American Heart Association (AHA) obtained prior to hire date or job transfer date required.
Licensed Registered Nurse credentialed from the Wisconsin Board of Nursing or current home state license for multi-state license recognition "Compact State" obtained prior to hire date or job transfer date required.
Education:
Graduate of a board-approved Registered Nurse Program required.
Bachelor of Science in Nursing (BSN) preferred.
Work Experience:
Ability to manage complex workload required.
