    What Could Your Day Look Like?

    You'll travel throughout the St. Paul area, completing skilled nursing visits that may include:

    • Comprehensive RN assessments
    • Skilled home health nursing visits
    • Wound care and wound assessment
    • Medication reconciliation and management
    • Injections and other skilled nursing procedures
    • Monitoring acute and chronic medical conditions
    • Patient and caregiver education
    • Assessing progress following hospitalization or rehabilitation
    • Identifying and communicating changes in condition
    • Coordinating with physicians and other healthcare providers
    • Collaborating with PT, OT, SLP, social work, and interdisciplinary team members
    • Helping prevent avoidable emergency department visits and rehospitalizations
    • Completing timely documentation in Homecare Homebase (HCHB)

    Who We're Looking For

    We're looking for an RN who likes a little movement in their day.

    Someone who enjoys meeting different people.

    Someone who can walk into a new environment, assess what's happening, and confidently determine what needs to happen next.

    You'll need:

    • An active Minnesota Registered Nurse (RN) license
    • Previous nursing experience in home health, hospital, acute care, rehabilitation, skilled nursing, or another clinical environment
    • Strong assessment and critical-thinking abilities
    • Confidence working independently in the field
    • Excellent patient and caregiver communication skills
    • Strong organization and time-management abilities
    • Ability to manage a daily schedule of patient visits
    • Reliable transportation and willingness to travel throughout the St. Paul service area

    Previous home health experience is valuable, but experienced nurses from other clinical settings are encouraged to connect with us.
