Registered Nurse - Hospice - Travel

Duties & Responsibilities:

• Performs patient assessments to determine appropriateness for hospice care, assess learning needs and progress toward goals, and develop plans of care for patients and families.

• Ensures appropriate utilization of services, maintains visit productivity standards, and demonstrates knowledge of insurance reimbursement guidelines.

• Completes discharge assessments, performs laptop data transfers twice daily, completes admission assessments and transfers within 24 hours, and completes and transfers revisit documentation.

• Uses age-appropriate teaching methods for patients and families, evaluates and revises teaching plans as necessary, and documents patient education tools used.

• Assumes responsibility for case management by:

  • Developing, implementing, and evaluating plans of care based on evidence-based knowledge.

  • Revising care plans as indicated and prioritizing patient care needs.

  • Demonstrating knowledge of hospice regulations and insurance reimbursement guidelines.

  • Maintaining interdisciplinary communication and communicating patient progress toward goals with the Interdisciplinary Team (IDT).

  • Performing Home Health Aide (HHA) supervision according to established standards.

  • Following scheduling processes and prioritizing schedules to accommodate patient needs.

  • Attending and participating in IDT meetings every two weeks.

  • Documenting case management activities.

• Maintains a safe environment for patients and coworkers, follows infection control measures, reports infections, demonstrates knowledge of pharmacology and safe medication management, routinely reconciles medications, and maintains current medication orders.

• Attends monthly staff meetings, participates in Performance Improvement (PI) meetings, activities, and committees as requested, and accepts other assignments as needed.

• Participates in the Performance Improvement Program by reviewing health records, gathering data to support PI activities, attending Utilization Review (UR) meetings as requested, and providing orientation and mentorship to new employees.

• Cross-trains to learn new skills, demonstrates willingness to teach others, and assists with agency growth by supporting new employee orientation and accepting additional assignments as needed.

  Required Skills & Experience:

  • Must have two (2) years of experience as RN in home hospice environment.
  • Experience using an EMR, excellent critical thinking and communication skills.
  • Three (3) + years of RN experience. - preferred

  Required Education:

  • Associate's (ASN) degree OR Diploma with RN License in New Jersey

  Required Certifications & Licensure:

  • Current New Jersey RN License
  • Must have valid New Jersey driver's license.
  • BLS.

  Shift:

  • 5 Shifts / 37.50 hours/week
  • 8a-4p
  • Every other weekend with comp days during the week
  • Can expect 4 - 5 visits per day as well as admissions.
  • Float: Yes to different counties according to need
