Position Overview:
Seeking an experienced Travel Registered Nurse to deliver compassionate, high quality end of life care to patients in the home setting. This role focuses on patient assessment, care coordination, and collaboration with interdisciplinary teams to ensure comfort, dignity, and quality of care for hospice patients.

Schedule & Work Details:

  • Location: Coverage across 6 counties, with attempts to assign a consistent geographic area based on census and daily needs
  • Schedule: Monday through Friday, 8:00 AM to 4:30 PM
  • Weekly Hours: 40 hours
  • On Call/Weekends: Required, rotating with team
  • Patient Load: Approximately 7 to 8 patients per day, may vary
  • Pay Rate: $51 to $53 per hour
  • Mileage Reimbursement: Yes, reimbursed between visits (not including commute to first visit or return home)
  • Dress Code: Preferred gray or black scrubs, non bright colors

Key Responsibilities:

  • Assess and manage hospice patient care in the home setting in accordance with care plans and physician direction
  • Perform medication reconciliation and coordinate medication refills as needed
  • Complete weekly Interdisciplinary Group (IDG) documentation and participate in IDG meetings
  • Communicate effectively with physicians regarding patient condition, changes, and care needs
  • Provide patient and family education on disease progression, symptom management, and care expectations
  • Maintain accurate and timely documentation using Homecare Homebase
  • Collaborate with interdisciplinary team members to ensure coordinated and compassionate care
  • Respond to on call needs and provide support outside of scheduled hours as required

Required Qualifications:

  • Active, unencumbered RN license in Tennessee or compact state
  • Recent hospice nursing experience required
  • Experience with Homecare Homebase (HCHB) documentation system
  • Ability to work independently as well as collaboratively with care teams and physicians
  • Strong clinical judgment and decision making skills
  • Ability to manage time effectively and prioritize patient needs
  • Valid driver’s license and reliable transportation

  • Strong organizational and communication skills
  • Ability to adapt quickly in a dynamic travel assignment setting
  • Experience balancing productivity while maintaining high quality patient care
