Location: East Baton Rouge, West Baton Rouge, Iberville Parish, Assumption Parish, Ascension Parish Schedule: Monday–Friday, 8:00 AM–4:30 PM
Weekend Rotation: Every 5th weekend Hours: 8-hour shifts, 5 days/week
Weekly Guarantee: 36 hours
Mileage Reimbursement: Yes — reimbursed between visits (first patient to last patient)
Dress Code: Any scrub color except neon
Our client is seeking an experienced Hospice Registered Nurse to join our compassionate interdisciplinary team. This role is ideal for a nurse who thrives in a field‑based environment, values autonomy, and is committed to providing exceptional end‑of‑life care.
Complete IDG notes, recertifications, and admissions
Provide daily patient visits and ongoing clinical assessments
Deliver high‑quality, patient‑centered hospice care in home and facility settings
Collaborate with the interdisciplinary team to support patient and family needs
Maintain accurate and timely documentation in accordance with agency standards
ASN (Associate of Science in Nursing)
Hospice experience required
HCHB (Homecare Homebase) experience preferred
Strong clinical assessment skills
Ability to work independently in the field
Additional hospice experience beyond minimum requirements
Strong communication and organizational abilities
Compassionate, patient‑focused approach to care
