Travel Registered Nurse (RN) – Home Health


Position Overview

Our client is seeking a skilled and reliable Travel Registered Nurse (RN) to provide high-quality, patient-centered care in the home setting across the Louisville Metro area. This role is ideal for an experienced home health nurse with strong clinical and documentation skills, particularly in OASIS and HCHB systems.


Schedule & Availability

  • Standard Schedule: Monday – Friday, 8:00 AM – 4:30 PM
  • Weekend Requirement: One weekend per month
  • No routine weekday on-call required unless otherwise specified

Coverage Area

  • Louisville Metro area

Key Responsibilities

  • Deliver skilled nursing care in accordance with physician orders and individualized care plans
  • Perform Start of Care (SOC), recertification, and discharge visits
  • Conduct ongoing patient assessments and monitor clinical progress
  • Complete accurate and timely OASIS documentation
  • Meet productivity expectations of 30 points per week
  • Coordinate care with interdisciplinary team members to ensure continuity of care
  • Educate patients and families on disease management, medications, and care plans
  • Maintain compliance with agency policies and regulatory standards

Qualifications (Required)

  • Active Registered Nurse (RN) license
  • Minimum of 1 year of home health experience
  • Experience using Homecare Homebase (HCHB)
  • Strong clinical assessment and documentation skills

Preferred Skills

  • IV therapy experience
  • Wound care expertise
  • Experience managing congestive heart failure (CHF) patients
  • OASIS certification
