Registered Nurse - Home Health

This applies to direct care staff (Examples: RN, LPN, Nurse Aides, Therapists) referred to Kentucky nursing facilities, assisted living communities, or long-term care facilities, in accordance with KRS 216.793. Duties: Provide skilled nursing care to patients in the home setting, performing comprehensive assessments, developing individualized care plans, and monitoring patient progress in accordance with physician orders.

Pride Health is hiring a Registered Nurse - Home Health to support our client's healthcare facility in Wallingford, CT. 
 
Job Details: 
Schedule: Monday–Friday | 9:00 AM–5:00 PM
Location: Wallingford, CT
Coverage Area: Groton, CT and Surrounding towns
Job Type: Local contract [Travel contracts are also available if you reside more than 50 miles from the specified location]
Contract Length: 14+ Weeks (Possibility to Extend)
Local Pay Range: $50.00 - $56.00/hour (W-2) Plus Mileage reimbursement
Mileage Reimbursement Rate: $0.50/ mile
*Pay offered is based on experience, expertise, credentialing, and education.
 
Duties: 

• Provide skilled nursing care to patients in the home setting, performing comprehensive assessments, developing individualized care plans, and monitoring patient progress in accordance with physician orders.
• Educate patients and caregivers on disease management, medication administration, infection prevention, medical equipment uses, and self-care techniques to improve health outcomes and promote independence.
• Maintain accurate clinical documentation, communicate changes in patient condition to the interdisciplinary healthcare team, and ensure compliance with agency policies, Medicare guidelines, and professional nursing standards.

Requirements: 

• License(s): Current and unrestricted RN licensure in state of practice
• Certification(s): Current CPR Certification or ability to complete within 90 days of hire 
• Years of experience: 1+ years of Home Health experience and Homecare Homebase (HCHB) is a plus
