Heralds Home Health is Hiring!
We are seeking a compassionate, experienced, and clinically skilled Registered Nurse (RN) to join our Home Health team serving patients throughout the South Bay area.
We are looking for a dependable nurse with strong clinical judgment, excellent assessment skills, and the ability to independently manage patient care in a home health setting.
Bilingual Spanish-speaking or Vietnamese-speaking nurses are highly encouraged to apply!
Key Responsibilities
- Perform comprehensive nursing assessments and evaluate patients' clinical conditions.
- Provide skilled nursing care in patients' homes.
- Identify changes in patient conditions and communicate concerns promptly.
- Develop, implement, and update individualized patient care plans.
- Perform wound care, medication management, and other skilled nursing procedures.
- Coordinate patient care with physicians, caregivers, and the interdisciplinary team.
- Complete accurate and timely clinical documentation.
- Educate patients and families regarding medications, disease management, and treatment plans.
Qualifications
- Active, unrestricted California Registered Nurse (RN) license.
- Strong clinical assessment and critical-thinking skills.
- Experience in Home Health, Hospice, Acute Care, or a related clinical setting preferred.
- Ability to work independently and make sound clinical decisions.
- Excellent communication, organizational, and documentation skills.
- Knowledge of OASIS documentation and Home Health regulations is a plus.
- Bilingual Spanish or Vietnamese preferred.
- Reliable transportation and willingness to travel throughout the South Bay area.
